eDocuments via Engaged MD
Fertility and Surgical Associates of California (FSAC) has partnered with EngagedMD to provide eLearning and eDocument resources to patients. The eDocument component of EngagedMD allows you to electronically complete your patient documents and sign consent forms using any web-enabled device when and where it is most convenient for you.
How eDocuments Work
Step 1
You’ll receive separate emails (patient and partner) from Fertility and Surgical Associates of California containing a link to review your patient documents or consent forms. You will be registered with EngagedMD at the time you make your New Patient Appointment. If you are not registered with EngagedMD, please call any one of our locations and the front desk staff will get you set up. In order to participate with EngagedMD through FSAC, you must provide a unique email address for both patient and partner. This is part of a multi-layered ID authentication process.
Step 2
These documents and forms have important information about your treatment and may require you to provide information (past medical history, etc.), make choices, initial or sign off on aspects of your procedure(s).
Step 3
Please note that when prompted to make decisions on a form, patients and their partners must select the same choices (if applicable) although you will be completing documents sent separately to individual emails.
Step 4
After completing the document, click “Finish” to pass it along to the next review/signer in the process. If you have any questions on documents/forms, do not hesitate to call someone from your FSAC clinical care team.
Once your New Patient documents are completed on-line, the office will be notified and this information will be used to help us prepare for your visit. Once all the required signatures are obtained on your consents, you will receive an electronic copy via email.
eLearning via Engaged MD
The eLearning component of EngagedMD provides educational videos to outline everything you need to know in order to make informed decisions about your treatment. The on-line videos enable you to learn at your own pace and in the comfort of your home so that you can come prepared to discuss the procedure(s) in detail with your physician prior to finalizing your treatment decisions.
How eLearning Works
Step 1
You’ll receive an email from FSAC with a link to the EngagedMD patient education center. You will be registered for use with EngagedMD at the time you make your New Patient Appointment. Step 2
Click the link in the email to set your password. The website is secure and encrypted, so you can be sure your healthcare information is protected. In order to participate with EngagedMD through FSAC, you must provide a unique email address for both patient and partner. This is part of a multi-layered ID authentication process.
Step 3
Your user name will be your email address and you can always log in using the link that was sent to your email.
Step 4
Complete your assigned module(s) prior to your next appointment. Most are optional with the exception of some assigned modules that must be viewed prior to your appointments. We will be tracking your progress from the office and sending reminder emails as needed.
